Job Description:
The Registered Nurse Team Leader is responsible for providing
guidance and leadership to the team who provides the services
pertaining to palliative and hospice care. The Registered Nurse
Team Leader is a professional member of the hospice
interdisciplinary team The annual salary range for this position at
commencement of employment is expected to be between $100,000 -
$115,000; however, pay offered may vary depending on multiple
individualized factors, including job-related knowledge, licensure,

RESPONSIBILITIES Provides the skilled nursing services necessary to
adhere to the patient’s plan of care developed by the
interdisciplinary team and the patient’s physician. Ensures the
family/patient is participating in the care planning process and
ensures appropriate documentation of same. Ensures adherence to the
plan of care for every patient/family assigned to the team. In
conjunction with the Hospice Quality Improvement Coordinator,
performs quality review of all chart documentation for patients
assigned to the team. Responsible for compliance with regulatory
agencies governing Hospice. Identifies the patient’s and family’s
physical, psychosocial, environmental, safety and developmental
needs and individualizes the patient’s plan of care based on the
needs identified. Assesses all aspects of the patient’s pain and
develops an individualized pain management plan. Anticipates
prevents and treats undesirable symptoms or secondary symptoms.
Identifies and addresses comfort care needs. Supports, instructs,
and educates the patient, family and caregiver. Documents problems,
nursing assessments, appropriate goals, care provided, care
interventions and patient and family outcomes achieved from
intervention and care provided. Coordinating all patient and family
services. Maintains the dignity of the dying patient. Supports the
patient’s and family’s unique spiritual and cultural beliefs.
Provides holistic, family-centered care across treatment settings
to improve quality of life. Consults and collaborates with the
interdisciplinary team and others involved in the patient’s care.
Performs duties consistent with the Nursing Practice Act including
the Standards of Competent Performance. Demonstrates the ability to
correctly use; direct the use of medical supplies and equipment; to
utilize and access both direct and indirect resources to ensure
patient safety. Instructs other health team personnel and the
patient's family, regarding the nursing care needs and other
related problems of the patient. Responsible for the care given by
the team, including the Hospice Aide, Licensed Vocational Nurse, RN
and other hospice team members to patients by supervising care as
required by law and licensure and recording supervisory visits on
the visit notes and/or joint visit forms as appropriate. Prepares
the Team On-Call report timely Participates in the On-Call
rotation. Participates in orientation, in-service education and
utilization review. Participates in patient care conferences,
including Interdisciplinary Group. Provides hospice in-services and
education to referring and residential facilities Prepares and
submits, as directed, all case documentation and required payroll
information on a weekly basis. Provides services within the
Agency's catchment area. Prepares re-certification and discharge
paperwork as appropriate and submits timely. Acknowledges patient
rights including privacy and confidentiality. Consistently displays
a professional appearance and conduct in all settings. Customer
service including interpersonal and interagency relationships are
to be positive and professional at all times. Assumes
responsibility and displays reliability for the completion of all
tasks, duties, communications and actions. Supports the mission and
goals of the Agency. Maintains professional growth and development.
Participates in performance improvement activities. Abides by
Agency policies and procedures. Accepts other duties as assigned.
EDUCATION, EXPERIENCE, AND TRAINING Licensed in the State of
California by the Board of Registered Nursing. Current CPR, health
clearance and clinical skills self-assessment. Successful passage
(80%) of Agency competency exam and core competency per skills
checklist. Minimum of one year of professional nursing experience
and BSN or one year of professional nursing experience within the
last 3 years Have the orientation and training appropriate to the
care of the hospice patient and his/her family.
